Urgent and Clinical Matters

This service is not suitable for urgent enquiries.

If you are unwell or have clinical concerns, please contact an alternative medical practitioner.

For medical emergencies, call 000 or attend your nearest Emergency Department.

Medical Records Requests

Medical records can only be released once we have received a patient-signed request from the receiving medical practice, in accordance with legal and privacy requirements.

Once a valid request is received, we are happy to facilitate a secure transfer of records.

• A Medical Care Summary can be transferred free of charge

• In circumstances where a full copy of medical records is required, an administration fee of $39 + GST applies

• If a full copy is requested, please email us and an invoice will be issued. Records will be transferred once payment has been received
