In order to provide you and your family with the best possible service, we believe it is necessary to be able to spend an appropriate amount of time with you, to be able to discuss your health concerns in an unhurried and comfortable environment. For this reason, we ask that all new patients book an 'Extended Consultation' with us. This will be discussed with you when you call to book your appointment.

In the interests of building a comprehensive picture of your current health situation, we ask all new patients fill out a 'New Patient Registration Form'. For this reason we ask that all new patients attend our Practice at least 15-minutes prior to the booked appointment time. Alternatively, the Form can be filled out in the comfort of your own home and brought to the Practice when you attend for your appointment. Copies of the form are available for download by clicking on the button below or by booking an appointment online, using the button at the top of the page.

Recalls and Reminders

As our Practice is committed to preventative healthcare, we may occasionally send you reminders regarding health services appropriate to your care. If you wish to opt out of this, please let us know.

Repeat Prescriptions and Referrals

Our Practice Policy is that all referrals and repeat prescriptions require a consultation.
